practices. Position Summary The Care Manager serves as a vital
member of the treatment team for foster and adoptive families. This
role focuses on developing and implementing Transitional Living for
Youth (TLVY) plans to support life skills development and
preparation for independent, self-sufficient living. The Care
Manager also oversees and consults on Positive Behavior
Intervention and Support (PBIS) instruction and framework
implementation. Staff members are expected to embody KVC’s core

Responsibilities Participate as an active member of treatment teams
and provide feedback on youth progress Attend Multi-Disciplinary
Team (MDT) and IEP meetings Travel to multiple counties and provide
services in family homes Develop and maintain TLVY plans and ensure
standards are met through accurate documentation Maintain
up-to-date youth records, including Ansell Casey assessments and
Transition Plans Provide de-escalation techniques, crisis
management support, and PBIS guidance Participate in a 24-hour
on-call rotation Facilitate supervised visitation and document
outcomes Collaborate with Department of Human Services and
community partners Conduct intake paperwork, progress summaries,
and quality assurance of charts Run and organize planning team
meetings for youth Ensure compliance with contract requirements,
timelines, regulations, and laws Provide culturally responsive and
sensitive services Observe and report signs of abuse or neglect
Attend required meetings, court hearings, trainings, and
supervision sessions Complete required ADP Learning and annual
training courses QualificationsEducation Bachelor’s degree from an
accredited college or university in a social service-related field
required. Experience Minimum two (2) years of experience in
children and family services preferred, including knowledge of
trauma-informed care. A Master’s degree in a related social
services field may substitute for required experience.
Licensure/Certification Valid driver’s license and auto insurance
required Licensure by the West Virginia Board of Social Work
Examiners (or equivalent) preferred but not required
