Job Description:

A mission-driven, not-for-profit healthcare organization in Newark, Ohio is seeking a part-time Nurse Practitioner to join a family practice team in a school-based health setting focused on expanding access to high-quality, patient-centered primary care in the local community.

Position Highlights:

• Part-time, school-based family practice setting
• Provide primary care services for adult patients
• Collaborate with physicians and interdisciplinary care teams
• Focus on preventive care, health education, and chronic disease management
• Participate in community outreach and population health initiatives

Responsibilities:

• Obtain comprehensive medical, family, and social histories
• Perform physical exams and develop treatment plans in collaboration with physicians
• Order and interpret diagnostic testing as appropriate
• Provide patient and family education to support health decision-making
• Coordinate care with other providers and community resources
• Document findings and care plans accurately in the medical record
• Participate in quality improvement and utilization initiatives
• Support health education and outreach efforts in the community

Requirements:

• Graduate of an accredited nursing program with master s preparation as a Nurse Practitioner
• Current Nurse Practitioner certification from a nationally recognized organization
• Active Ohio licensure or eligibility to obtain
• BLS and ACLS certification required
• Prior NP experience preferred
• Strong communication, collaboration, and clinical decision-making skills
• Ability to work effectively in a team-based care environment
