Available**Job Overview:**We are seeking a compassionate and
dedicated Licensed Practical Nurse (LPN) to join our healthcare
team. This part-time and PRN position offers flexible scheduling
options to accommodate your availability. The ideal candidate will
provide high-quality nursing care to patients, assist with their
daily living activities, and work collaboratively within a
multidisciplinary team to ensure optimal patient outcomes.**Key
Responsibilities:**- Provide direct patient care under the
supervision of a Registered Nurse or physician, including
administering medication, performing treatments, and monitoring
vital signs.- Assist in the development and implementation of
individualized care plans based on patient assessments.- Educate
patients and their families on health management, medication
administration, and self-care techniques.- Collaborate with
healthcare team members to provide comprehensive care and promote
patient wellness.- Maintain accurate and up-to-date patient
records, documenting care provided and any changes in patient
condition.- Execute physician orders and follow facility protocols
in adherence to state regulations.- Assist with admissions,
discharges, and transfers of patients as necessary.- Participate in
ongoing training and professional development to stay current with
nursing practices and regulatory requirements.**Qualifications:**-
Current and valid LPN license in [State].- Completion of an
accredited LPN program.- CPR and First Aid certification required;
additional certifications as needed.- Prior experience in a
healthcare setting preferred, but not required.- Excellent
communication and interpersonal skills.- Strong organizational
skills and attention to detail.- Ability to work independently and
as part of a team.- Flexibility to work various shifts, including
