Job Description:



``` - Provide guidance and social assistance to community residents - Complete SDoH screening to identify and prioritize the individual's SDoH needs. - Provide case management and follow-up to ensure that the linkages to services and supports addressed the participant's needs. - Provide outreach and education to local service providers, stakeholders and individuals with SDoH needs within Indiana County through presentations at social service meetings, telephone connections with potential referral agents and the appropriate use other communications mediums for educating the community on these services. - Attend necessary trainings and meetings as required; travel required. - Promote practices that support equity, diversity, inclusion, and cultural humility. - Other duties and projects as assigned.

Candidates must meet the following requirements for consideration:

Bachelor's degree in related field preferred, or commensurate experience, previous experience in the human services, and social work preferred.
Project coordination and organization skills; ability to manage multiple projects with attention to detail; meet deadlines, ability to handle interruptions, and produce timely, accurate work.
Previous experience in a community health or related role.
Excellent written and strong communication and interpersonal skills.
Good listener, thoughtf l respondent and positive influencer.
Ability to work independently and as part of a team.
Excellent organizational and time management skills.
Demonstrated ability to communicate effectively with all members of the community.
Working knowledge of computers, including MS Word, Excel, and be willing to learn and utilize agency software programs such as ORS, as they pertain to this position.
Current resident of Indiana County preferred or individual with strong personal and/or geographic connections to the communities served.
Able to work evenings and weekends as needed to support the achievement of all project goals and objectives.
Must possess a vehicle and valid driver's license. Work travel, if required, is reimbursed at the federal reimbursement rate.

Must have or be willing to obtain Act 33 & 34 clearances and FBI clearance with fingerprinting.
Must be willing to obtain CHW certification; course to be provided by Employer. Must pass course or successful completion of course.
