-Responsibilities:
- ASSESSMENTS: Completes ongoing assessments.
- Recognizes significant changes in patients condition and
responds appropriately.
- PLANNING: Develops measurable goals for identified
problems.
- Updates goals for identified problems.
- Involves the patient/family (or designee) in developing a plan
of care individualized to patient cultural, religious and
educational needs.
- Coordinates the interdisciplinary plan of care.
- Serves as a patient advocate.
- Collaborates with multidisciplinary team to formulate an
individualized plan of care.
- IMPLEMENTATION: Provides nursing care based on patient-centered
outcomes and standards of care that are based on current research
and evidence-based practice.
- Safely administers patient care and treatments in accordance
with the Standards of Safe Nursing Practice and institutional
policy.
- Documents legibly using concise terms, correct terminology and
approved abbreviations.
- EVALUATION: Participates in on-going interdisciplinary
evaluation of patient/family outcomes and contributes to the
improvement of care.
- Participates in the quality improvement processes.
- LEADERSHIP/PROFESSIONAL DEVELOPMENT: Delegates
tasks/assignments to employees according to their ability and
within the scope of their job descriptions and according to the
Delegation by Licensed Nurses, Ohio Administrative Code.

- Completes mandatory education and training in order to maintain
organization and department specific competencies and

- Maintains applicable certification/licensure. Maintains BLS.
Completes psychiatric core curriculum and managing disruptive
behavior training with 6 months of appointment.
- Follows hospital and department policies and procedures with
special attention to attendance and punctuality, confidentiality,
dress code and display of ID badge, and safety.
- DEPARTMENT SPECIFIC DUTIES: Provides individualized and timely
interventions in accordance with age specific needs and Department
of Nursing/hospital policy including establishing priorities for
delivery of patient care to individual patients and for assigned
group of patients. This also includes remaining calm and
professional during crises or emergency situations.
- Accurately documents nursing interventions and patient's
responses, in accordance with policy.
- Participates in and documents patient care and discharge
planning (including progress toward goals) including working with
care management staff to facilitate quality patient care with
effective use of resources throughout the continuum.
- Demonstrates familiarity of unit-based and hospital QI
activities and participates in QI data collection.
- Communicates information regarding patient's plan of care,
current outcomes, and acuity at shift report to oncoming nurses and
support staff.
- Using appropriate chain of command, communicates information in
a timely fashion to Director, physician, or Nursing Coordinator
regarding changes in patient's condition and patient care/unit
issues.
- Demonstrates ability to perform unit coordinator
responsibilities as assigned.
- Participates in clinical instruction programs to provide
learning experiences for students and participates in orientation
of new team members.
- PSYCHIATRIC TRIAGE NURSE DUTIES: *ED triage nurse may work as a
psych staff RN when needed - (Director/Manager discretion).
- Triages psychiatric patients that present to the ED, and
completes the "Medical Screening Examination" form. Initiates
admission paperwork, assists with insurance/pre-certification,
communicates with attending physician and obtains orders, gives
report to the floor, documents all pertinent clinical information,
and assists with transporting the patient.
- Assists with assessment and treatment of psychiatric nursing
issues in the ED, including consultation on managing disruptive
behavior and restraint issues. This includes assessing the patient
for suicide and elopement risk and recommends proper level of
patient observation. May perform nursing consultation in inpatient
areas.
- Reviews the case with the patient's psychiatrist or appropriate
psychiatrist on call as well as the ED physician and collaborates
with the Psychiatric Admission Coordinator and Urgent Behavioral
Evaluation Team as appropriate
- Provides discharge instructions for those who are treated and
released.
- Serves as a resource/liaison for referrals for admission to
psychiatry who are not in the emergency department such as
community mental health centers, other hospitals, or
facilities.
- Other duties as assignedEducation:
- Graduate from an accredited school of nursing BSN
preferred.Certifications:
- Currently licensed as a registered nurse in the State of
Ohio.
- BLS requiredComplexity of Work:
- Maintains competencies specific to patient population
served.
- Satisfactorily completes psychiatric core curriculum and
disruptive behavior training within six months of appointment.Work
Experience:
- If assigned to ED Triage primarily must have five years nursing
experience with three years in inpatient psych.
- One year of Psychiatric nursing preferred.Physical
Requirements:
- Requires full range of body motion including handling and
lifting patient, manual and finger dexterity, and eye-hand
coordination.
- Requires standing and walking for extended periods of
time.
- Requires corrected vision and hearing to normal range.
- Requires working under stressful conditions and irregular
hours.
- Exposure to communicable diseases and/or body fluids.
- Medium Work - Exerting 20 to 50 pounds of force occasionally,
and/or 10 to 25 pounds of force frequently, and/or greater than
negligible up to 10 pounds of force constantly to move
objects.
- Physical Demand requirements are in excess of those for Light
