Job Description:
General Description:The Population Health Quality Coordinator
assists in facilitating high-value, patient-centered care that
improves timely access to and provision of preventive services and
chronic disease management. This will be accomplished by
implementing processes to improve the targeted measure of
Colorectal Cancer Screening. The Population Health Quality
Coordinator will support the providers and care teams using a
population management informatics registry tool to identify the
targeted population. The registries will be used to identify
patient care needs, by gathering and organizing patient data and
implementing directed interventions to improve quality of care.
Duties:
- Utilize patient registries and provide members of health care
teams in designated practices, and AxCS Nurse Navigator with the
required data to meet the health needs of the patient.
- Provide data management, coordination and patient outreach for
specific target patient populations.
- Coordinate patient care for colorectal cancer screenings, which
may include:o Providing patient education, preparing lab or test
orders, mailing testing supplies, fax and/or scheduling referrals
with specialty offices, providing designated patient records to
specialty offices, coordinating transportation for appointments,
tracking & obtaining completed service records, updating the
electronic medical record flow sheet and scanning documents into
charts.
- Conduct outreach activities to patients regarding appointments,
health maintenance, etc. through phone communication, email,
letters, or other mailings.
- Communicate with the pay for value team to help facilitate in
meeting the needs of patients for the value-based programs.
- Communicate with site leadership when centralized outreach
attempts fail to engage the patient.
- Consult with Community Health Workers as needed, to assist in
overcoming barriers for the patients to access necessary care.
- Communicate with the primary care provider and other team
members
Qualifications:
- High Scholl Diploma or GEDMinimum of 2 years of experience in a
health care
o Preferably with clinical background
- Proficient in data management and reporting
- Excellent organizational skills and problem-solving skills
- Strong orientation to detail
- Proficiency in electronic health record documentation and
electronic chart review
- Knowledge of computer-based programs such as Word, Excel and
Outlook
- Excellent communication skills
- Flexible Understanding of the process improvement methodology
preferred
