Job Description:
- Evaluates patient's clinical status and provides recommended
rehab services as per protocol.
- Utilizes assessment skills in providing patient care in all
phases of cardiac rehabilitation.
- Educates patients/families about cardiopulmonary diseases and
treatments.
- Participates in case management, discharge planning conferences
with other interdisciplinary staff.
- Documents patient progress and collaborates with various
members of the health care team to meet patient needs.
- Develops rehabilitative goals to prevent progression of heart
and lung disease (exercise prescription, activities of daily
living, risk factor reduction, prevention of exacerbations,
etc.).
- Assists in coordinating patient care and developing new
programs, educational materials and policies/procedures.
- Assists in developing and evaluating performance improvement
standards.
- Remains flexible in working between all phases of
rehabilitation, cardiopulmonary procedures and as indicated in the
Heart Center.
- Attends continuing education programs to facilitate
professional growth.
- Supports and participates in community wellness programs.
- Serves as preceptor for university students in nursing, health
and exercise physiology.
- Promotes and assists in marketing strategies for the promotion
of outpatient cardiopulmonary rehabilitation programs.
- Maintains patient records and statistics.
- Completes mandatory education and training in order to maintain
organization and department specific competencies and requirements.
Maintains applicable certification/licensure.
- Remains flexible to work in and cross train between other areas
in the Heart and Vascular Center.
- Supports and participates in community wellness programs.
- Serves as preceptor for university students in nursing and/or
other health sciences.
- Other duties as assignedEducation:
- Graduate of an accredited school of nursing. Bachelor's Degree
in related health field and/or Master's Degree.Certifications:
- Registered Nurse with current license to practice in
Ohio.Complexity of Work:
- Maintains competencies specific to patient population
served.
- Knowledge of exercise physiology preferred.
- Advanced Cardiac Life Support (ACLS) preferred.Work Experience:
- Two years nursing experience in coronary care critical care
and/or medical/surgical patient care unit.
- One year cardiac and/or pulmonary rehabilitation experience
preferred.Physical Requirements:
- Requires full range of motion, manual and finger dexterity and
eye-hand coordination.
- Requires standing and walking for extensive periods of
time.
- Requires corrected hearing and vision to normal range.
- Requires some exposure to communicable disease or bodily
fluids.
- Medium Work - Exerting 20 to 50 pounds of force occasionally,
and/or 10 to 25 pounds of force frequently, and/or greater than
negligible up to 10 pounds of force constantly to move objects.
Physical Demand requirements are in excess of those for Light Work.
Personal Protective Equipment:
- Follows standard precautions using personal protective
equipment as required Under the direct supervision of the Director,
Cardiopulmonary Rehabilitation, the Cardiopulmonary Rehab Nurse
Specialist is responsible for planning, coordinating, and
implementing all aspects of therapy for cardiac and pulmonary
patients. The Specialist assists the Director in achieving
departmental objectives.
