between the community and the health care, government, and social
services systems to provide clients with the psychosocial support
needed to cope with chronic, acute, or terminal illnesses.
Collaborates with other professionals to evaluate patients' medical
or physical condition and to assess client needs. Advocates for
clients or patients to resolve crises. Facilitates access to
resources and services to improve the quality and cultural
competence of service delivery. Performs home visits, provides
health screenings, and delivers education presentations as
necessary. Formal Education Required:a. High School Diploma or
equivalentb. Associates Degree, preferredExperience & Training
Required:a. Certified as a Community Health Worker through Ohio
Board of Nursingb. Two (2) years of experience as a Community
Health Worker Other Skills, Competencies and Qualifications:a.
Certification through Ohio Board of Nursingb. Basic computer
skillsc. Must pass background check and drug testd.Knowledge of
basic medical terminologye. Ability to work with diverse
communities and different races, cultures and classesf. Ability and
willingness to provide emotional support, encouragement and
motivation to clientsg. Ability to effectively interact with
populations of patients/customers with an understanding of their
needs for self-respect and dignityh. Ability to obtain and record
vital signs, and recognize changes in health statusLevel of
Physical Demands:a. Light: Exerts up to 20 pounds of force
occasionally and/or up to ten pounds of force frequently, and /or a
negligible amount of force continuously.
